What an NCR contains
NCR number and date, item and batch, where it was found (incoming, in-process, final, customer), the requirement and what was found, quantity affected, photos or measurements, disposition, and who approved it.
Disposition options
Use as is (with approval), rework, repair, return to supplier, or scrap. If the cause could recur, the NCR is escalated to a CAPA so the root cause is fixed, not just the batch.
Example
Incoming inspection finds 40 of 500 castings with blow holes: NCR raised, 40 returned to the supplier, CAPA opened with the foundry.
